Requirements and agreement

If my foster needs to be seen by a veterinarian, I will get pre-approval from the director.
If my foster animal is seen by a veterinarian and I request additional medical services without pre-approval, I assume responsibility for payment.
If I take my foster animal to an unauthorized rescue veterinarian for Salvaged Souls Pet Rescue, I assume responsibility for payment.
I am responsible for getting all documentation, including shelter paperwork, medical records, and vet visits, sent to the director in a timely manner.
I understand that information and vet visits must be logged under Salvaged Souls Pet Rescue as the owner of my foster animal.
I agree to keep the director updated on my foster animal’s well-being and to send presentable photos and updated information.
If I want to be considered as an adopter for my foster, I agree to follow Salvaged Souls Pet Rescue's standard adoption process, including paying the required adoption fee.
If my foster dog needs grooming, I will contact the director so the foster dog can be scheduled at the rescue's place of choice under the name of SSPR.
I will not have my foster dog shaved down without consulting the director of SSPR.
I understand my foster dog is not allowed to leave the DFW area without approval from the Salvaged Souls Pet Rescue director or a board member.

I understand that applying does not ensure approval, and that untruthful answers or failure to comply with these requirements can result in forfeiture of any Salvaged Souls Pet Rescue animal fostered by me. I certify that the information is correct and may be verified; I release and hold harmless Salvaged Souls Pet Rescue and its members from claims or damages arising from fostering; I accept full responsibility for the dog’s actions; I agree to reference checks and a home visit; and, if I can no longer foster, I will return the dog to Salvaged Souls Pet Rescue and give the rescue two weeks to find a suitable foster whenever possible.
